Teachers and day care provider often want to take their youthful charges on escapades. However, you'll need parent's permission first.

Steps

  1. Start by writing a WHO: WHAT: WHEN: WHERE: WHY: and HOW: at the top of your Word document. Who should be who will be going on the trip, what will be what to take with you, when will be the date, where will be the location(s), why will be the reasons for the trip, and how will be how you will be transported.
  2. Next, provide a blank (_____) and say _____, I the undersigned give permission for my child, ______, to attend the field trip with ______ (your organization.
  3. Now, add blanks for the child's full name, date of birth, sex, allergies, medical conditions, and health insurance provider.
  4. You'll want the number of their doctor, and the numbers of their parents and an alternate emergency contact.
  5. Add space for a parent's signature at the bottom, and you are done!

Tips

  • Make sure you get your supervisors permission before taking a trip.
  • Go to Kinko's for copies, it's cheaper than using your own ink.

Warnings

  • Make sure you have enough chaperons!
